
Mobile Crisis Response
Our Mobile Crisis Response service provides rapid, on-site mental health intervention for individuals experiencing an emotional, behavioral, or psychiatric crisis. Services are delivered in the community—including homes, schools, workplaces, shelters, or other safe locations—to stabilize the situation and reduce the need for hospitalization or law enforcement involvement whenever possible.
What We Do
Our licensed and trained crisis clinicians respond quickly to assess risk, de-escalate crises, and support individuals and families during moments of acute distress. Services are trauma-informed, person-centered, and culturally responsive.
Mobile Crisis Services Include:
Community-Based Crisis Stabilization Program
Community-Based Crisis Stabilization services are available 24 hours a day, seven days a week, to provide short-term assessment, crisis intervention, and care coordination to individuals experiencing a behavioral health crisis.
Services may include brief therapeutic and skill-building interventions, engagement of natural supports, and interventions to integrate natural supports in the de-escalation and stabilization of the crisis. Services also involve advocacy, networking, and coordination of follow-up services, including assistance accessing benefits and other support programs.
Utilizing a person-centered approach and collaborative decision-making, this program is guided by professional-level counselors working as a team to meet individual service plan objectives.
RTC office hours are Monday–Friday, 9:00 AM–5:00 PM, with 24-hour on-call emergency service.

Rebuilding Transformation Center, LLC (RTC) provides Mental Health Skill-Building Services (MHSS), a community-based service designed to deliver structured, individualized interventions that restore, develop, and maintain independent living skills.
Services are focused on improving functional ability, promoting stability, and supporting long-term independence within the community.

strengths, needs, and goals.

Our Services
Rebuilding Transformation Center, LLC (RTC) provides structured, individualized, and goal-directed Mental Health Skill-Building Services (MHSS) designed to restore, develop, and maintain independent living skills. All services are person-centered, recovery-oriented, and directly aligned with each individual’s plan of care.
Skill-Building Interventions
- Activities of Daily Living (ADLs), including hygiene, grooming, nutrition, and self-care
- Instrumental Activities of Daily Living (IADLs), including budgeting, transportation, and household management
- Medication management support (non-clinical), including education and adherence support
- Financial management and budgeting skills
- Time management and routine development
Behavioral and Emotional Supports
- Coping skills development
- Crisis prevention strategies and early intervention techniques
- Social skills training and interpersonal effectiveness
- Problem-solving skill development and decision-making
Community Integration
